Dental Patient Forms
We ask that you please fill out your new patient forms before you come to our office for your first appointment, as this will help to streamline your visit and help you avoid needing to do so in our waiting area. You can download them by clicking on the link below, which will allow you to print them and fill them out at home. If you have difficulty accessing them, please call our office.
Dental Insurance
We’re proud to be in-network with several major PPO dental insurance plans, and we’re happy to file claims on your behalf, even if your insurance isn’t in-network with our practice. That way, we can help you get the maximum reimbursement amount. We also have several insurance experts on our staff, so if you have any questions or concerns about your coverage, we encourage you to ask us!
CareCredit Financing
For those who are uninsured or underinsured, or for our patients who will be receiving treatments that aren’t covered by their benefits, we also offer additional third-party financing through CareCredit. Upon approval, you’ll be able to finance your treatment at little-to-no interest, allowing you to make small, more manageable monthly payments on the overall cost.
